GFGL Tarkwa Gold Mine Incident Analysis

The following charts and tables present the results of the incident analysis conducted as a part of the VICE Baseline validation process.  Key observations by Risk Mentor of the incident data are:

  • The total number of reported incidents is significantly lower than other, similar sized, mining operations – which may be an indication of under-reporting of near miss type events,
  •  The higher proportion of ROS 01 – Vehicle Operators Maintain Adequate Clearance indicates that reports are dominantly being made following equipment damage,
  • Failed Equipment components represent a higher fraction of total incidents that observe at other, similar, mine sites,
  • No unique failure modes were identified for Tarkwa Gold Mine – which indicates the control framework model is applicable for the site.
This graphic shows a large number of the incident relate to vehicles contacting other vehicles and objects. This percentage is higher than typical for mining operations.

HME_001 – Dept Mining Maintenance – Area New Access Gate 5/3/2022 While driver of Hiab TK 132 (GC 1506-18) was moving off at the New Access Gate, the boom incidentally lowered down and made contact with the tail end of the Hiab and resulted in damage to the boom. MIN_007 – Dept Mining – Area Teberebe west waste dump 5/4/2022 On Tuesday,  5th  April, 2022, at about 11Hrs10, a team of surveyors  in LV GW 6481-16 was descending the Teberebie West Waste dump ramp whilst an oncoming  empty dump truck (DT284) from the waste dump flipped a chip of rock onto the windscreen. SCM_001 – Dept Supply Chain – Area Phase V 8/4/2022 A  Ferric delivery truck on its way to North Water Treatment plant fell into a pothole causing the load to balance and the hook holding the last pallet broke off. This resulted in one drum of Ferric falling off. MIN_010 – Dept Mining – Area One-way access to New Teberebie chophouse 19/5/2022 The driver of LV 381 veered off the road while he was on the road to the new Teberebie chophouse. The Position one tyre of the vehicle climbed the left side windrow and fell on the right side as depicted in the picture below. There was damage to the vehicle but no injury. ENG_006 – Dept Engineering – Area High table Catering Services 23/5/2022 The driver of Kia truck with registration number GM1085-13 (owned by African Horizon Limited) reversed over the concrete stopper/berm at mine’s  restaurant, in an attempt to drive out of the parking lot. ENV_002 – Dept Environmental – Area Teberebie West WRD Ramp 24/5/2022 Whiles descending the Teberebie West Waste Rock Dump ramp towards S-Curve, the driver of skip truck with registration number GR 587-17 heard an unusual noise at the position 2 hub. In an attempt to park at a safer place the hub buckled and the position 2 tyre got detached. ACC_002 – Dept Accra Office – Area Accra 1/6/2022 Osman, on his way to the Corporate Office from the Accra International Conference Centre whilst driving vehicle number GN 503 – 21 on the Kanda Highway, hit the rear of a Toyota Belta with registration GX 2646-22. This was as a result of the Belta applying his brakes immediately as a vehicle without caution moved into the lane of the Belta. The case has been lodged with the Airport Police Station and investigations are underway. MIN_011 – Dept Mining – Area AECI Ramp 2/6/2022 A truck with registration number GT 8464-16 lost engine power whiles ascending the AECI ramp. The driver managed to bring the truck to a stop. Field dug and alcohol tests conducted on the driver were negative. MIN_012 – Dept Mining – Area HME Back Boom Gate 10/6/2022 The driver of LV with registration number GT 9703-18 belonging to Twin Rock in attempt to stop and swipe at the HME back boom gate, had the bonnet of his LV coming into contact with the boom gate. The contact resulted in damage to the boom gate. PS_003 – Dept Protection Services – Area CIL Goods Air Lock 6/7/2022 John was assigned to pick up samples from the CIL plant. Immediately he drive through the gate with LV GT 3513 Y, to the goods airlock, the glass of the right back door shattered. MRM_001 – Dept Mineral Resources – Area Tarkwa North( Bekyere Road) 9/7/2022 Driver of LV # GW 4759-17 skidded of the road and hit  rubber tree. The incident occurred whiles he was returning from the exploration site. PS_004 – Dept Protection Services – Area Central Workshop 18/7/2022 Samuel Larbi whilst existing from the Central Workshop yard to escort WTS bus into the pit made contact with the right side of the main gate causing damage to the gate and a dent on the right fender of LV  # GW 4510-16. Samuel claimed that he got distracted by an insect flying into his eye. HME_002 – Dept Mining Maintenance – Area HME Go-Line 22/7/2022 On the 22-07-22 at about ±1235 hours GMT, the driver of LV 362 belonging to BCM reversed into LV (GT 9320-21) belonging to E&P at HME ‘Go Line’ thereby denting the bonnet of E&P LV as depicted in the picture below. SCM_002 – Dept Supply Chain – Area Weigh-In Bridge 2/8/2022 The Driver of LV 382 while existing through the HME back gate ran through the boom gate without stopping. The boom was still elevated after a vehicle in front of him had passed. Ref 64921 Mining – Pepe Footwall Dump on 24/06/24 at 9:06 24/6/2024 BCM Truck 215, hauling waste material was in the process of reversing to dump at the Pepe Foot Wall Dump when the tip head failed. The truck subsequently slid down the dump face for approximately seven (7) meters. The downward movement was arrested by the rear tyres of the truck buried in the slumped material. The truck had not yet commenced to tipping and was not intending to tip over the berm but in front of the berm as per the procedure.
